چکیده

between CVP and indexes of fluid responsiveness might be influenced by ongoing fluid losses or gains that were not necessarily easily measurable. Finally, in many clinical situations, the success or failure of fluid resuscitation measures is not determined by preset goals in terms of cardiac output or other indexes of cardiac performance but by criteria such as pressor requirements or blood lactate concentrations. While the current recommendations guiding fluid replacement use CVP as only one of a number of clinical parameters to follow,4 and while dynamic parameters may have an increasing role in guiding fluid replacement,5 it may be premature to discard CVP measurements in ICU patients.

Challenges associated with central venous catheter placement and central venous oxygen saturation monitoring.

Patients with septic shock and respiratory failure often require hemodynamic monitoring in order to guide bedside management. This is particularly true of patients with dialysis-dependent chronic kidney disease, who may be less tolerant of large volume resuscitation than individuals with normal renal function. With the declining use of pulmonary artery catheters and the introduction of early go...

Determination of Central Venous Pressure from Intraocular Pressure

Background: In many clinical situations, knowledge of central venous pressure (CVP) is important.  Measurement of CVP is not always possible. Objective: To determine if intraocular pressure (IOP) can be used as an estimate for CVP. Methods: IOP and CVP were measured concurrently in 30 candidates of coronary artery bypass graft surgery.  Those with carotid artery or jugular venous diseases as we...

The efficacy of inferior vena cava diameters and the jugular vein in assessing fluid resuscitation

Objective: Fluid resuscitation is necessary in almost all critical patients. The central venous pressure (CVP) is a well-established method of assessing resuscitation. Recently, there have been attempts to investigate less invasive methods like the diameters of inferior vena cava (IVC) or the jugular vein. We aimed to investigate this method in our research. Methods: Seventy eight critical...

Mean arterial pressure: therapeutic goals and pharmacologic support.

The Surviving Sepsis Campaign targets central venous pressure, mean arterial pressure, and central venous oxygen saturation as guides for resuscitation. Fluid resuscitation and the use of vasopressors are paramount to the success of the campaign's end points. Although the achievement of supranormal physiologic parameters has been associated with higher mortality in some studies, these slightly ...

Starling curves and central venous pressure

Recent studies challenge the utility of central venous pressure monitoring as a surrogate for cardiac preload. Starting with Starling's original studies on the regulation of cardiac output, this review traces the history of the experiments that elucidated the role of central venous pressure in circulatory physiology. Central venous pressure is an important physiologic parameter, but it is not a...
